By accessing and using the Flore CLINICAL Portal ("Service"), you agree to be bound by these Terms of Service. If you do not agree, you may not use the Service.
Flore CLINICAL provides a platform for licensed healthcare providers to access personalized probiotic formulation recommendations based on patient microbiome analysis, lab results, and clinical symptoms.
You must be a licensed healthcare provider in good standing. By registering, you represent that you hold a valid professional license, are authorized to recommend nutritional supplements, and will only use the Service for legitimate clinical purposes.
You are responsible for obtaining appropriate patient consent before entering data. All patient data is encrypted and handled in accordance with HIPAA regulations. For full details, see our Privacy Policy.
The Service provides formulation recommendations based on algorithms. These do not constitute medical advice. You retain full clinical responsibility for all treatment decisions.
All formulations, strain combinations, and algorithms are exclusive intellectual property of Flore, Inc. You may not reverse-engineer, copy, or disclose formulation details.
Flore CLINICAL is provided "as is" without warranties. Flore, Inc. shall not be liable for any indirect, incidental, or consequential damages.
These Terms shall be governed by California law. Disputes shall be resolved in San Francisco County courts.
At Flore, Inc., we are committed to protecting your privacy and the privacy of your patients.
We collect provider information (name, email, credentials) and patient health data you enter for formulation purposes.
Data is used solely to provide personalized probiotic formulations and improve our clinical algorithms. We never sell personal information.
All data is encrypted using AES-256 encryption at rest and TLS 1.3 in transit. We maintain SOC 2 Type II compliance.
You may request access to, correction of, or deletion of your data at any time by contacting support@flore.com.
This Agreement is between Flore, Inc. ("Business Associate") and you, the healthcare provider ("Covered Entity").
Business Associate agrees to: Not use or disclose PHI except as permitted; Use appropriate safeguards to prevent unauthorized disclosure; Implement administrative, physical, and technical safeguards; Report any unauthorized use or disclosure; Ensure subcontractors agree to same restrictions.
Business Associate may use PHI for: Performing services under this Agreement; Management and administration; Data aggregation for healthcare operations; Reporting violations of law.
Business Associate implements: AES-256 encryption for all PHI; Role-based access controls; Comprehensive audit logging; Regular security assessments; SOC 2 Type II compliance.
Business Associate shall report any breach of unsecured PHI within 60 days of discovery.
This Agreement remains in effect during your use of the Service. Upon termination, Business Associate shall return or destroy all PHI.
This Agreement establishes terms of your participation in the Flore CLINICAL Provider Network.
As a participating provider, you agree to: Maintain current licensure; Obtain informed consent from patients; Review all formulation recommendations before ordering; Monitor patient outcomes and report adverse events; Complete required training.
You acknowledge that: All clinical decisions remain your sole responsibility; Flore formulations are supplements, not drugs; Results may vary; You will not make unsupported claims.
To improve formulations, you agree to: Record patient outcomes when requested; Participate in optional clinical research; Allow de-identified data for research.
Provider pricing is confidential. You may set your own retail pricing. Payment is collected at time of order.
You agree to indemnify and hold harmless Flore, Inc. from any claims arising from your use of the Service.
Orders may be cancelled within 24 hours of placement, before manufacturing begins. Once manufacturing has started, orders cannot be cancelled due to the personalized nature of each formulation.
If a patient's current formula isn't meeting their needs, you may request a reformulation under these conditions:
Due to the personalized nature of our products, we cannot accept returns of opened products. If a patient is unsatisfied after 30 days of consistent use, contact us to discuss reformulation options.
If you receive a defective or damaged product, contact support@flore.com within 7 days for a replacement.
Flore CLINICAL improves formulation algorithms through clinical research. By participating, you consent to:
We may use de-identified clinical data for: Algorithm improvement and machine learning; Clinical outcome research; Publication in peer-reviewed journals; Regulatory submissions.
All research data is de-identified per HIPAA Safe Harbor standards. No identifying information is included.
Research activities are conducted under IRB oversight where applicable. Protocol #2026-FLORE-001.
You have the right to: Request a copy of your data; Withdraw from research (prospectively); Receive notification of published research; Contact our research team.
